At each exemplification, we encounter form 4's which have been incorrectly
filled out. To alleviate this situation, instructions are provided below
on how to correctly fill out the form 4's:
Below is a form 4 PDF file which can be filled out electronically and printed.
Once the form 4 has been correctly filled out, with the Financial
Secretary and members signature, it can be delivered to the assembly
of the members choice Navigator to be read and voted on. After the assembly votes
on the new candidate, the Navigator will send the form 4 to the District Master.

candidates for the Fourth Degree:
